Complete this form and call us at 905-454-8725 to schedule an interview with our Volunteer Coordinator. Please bring this sheet in with you to your interview.

VOLUNTEER AGREEMENT
All volunteers and volunteer applicants must consent to this agreement to be eligible to volunteer at the Knights Table.
